Our assistance springs from our initial consultation. There is no charge for an initial consultation which is typically 15 to 30 minutes long. Care Decisions
Providing and coordinating care for an older adult can be complex. We can assist you in developing a care plan, provide strategies for making changes when necessary, and help you make informed decisions that best meet your needs and concerns. Sometimes it just helps to talk it through with someone who has been there and knows how the system works. Awareness of appropriate and alternative choices helps you make the best decisions. You don’t have to do this alone.
Crisis Management and Advocacy
A sudden medical event can cause choices to be made quickly without having time to explore options. Accurate and timely information is critical to the care team and the family decision makers. We can support the family decision makers in dealing with the medical team. Knowledge of how the system works and what your rights are both as a patient and as a decision maker is crucial to getting the best care with the most favorable outcomes. We can also assist in helping you develop an immediate and short-term care plan, followed by an assessment of longer-term goals and care options.
Aging in Place
Over 80 percent of seniors would prefer to age in place rather than move in with family or to a senior living residence/community /care facility. The biggest hurdle in remaining in their home is getting support structures and programs in place at home to make it feasible. That’s where we make a difference. We will assist you in creating an age-friendly environment. With our knowledge of available programs and services, we can help you with setting up needed personal supports so seniors can stay at home for as long as possible, if that is their choice and it is safe to do so.
Dementia Care Coaching
It’s important to acknowledge the waves of grief and loss family members and friends feel when caring for a loved one with dementia.
We can help you:
- Understand the progression of the disease
- Implement real solutions to reduce stressors for the caregiver/ family and the person diagnosed with dementia.
- Discover how best to communicate to the person with dementia to avoid negative behavior triggers
- Learn how to manage difficult issues such as the refusal of medications, showering, changing clothing, and more.
Let’s work toward creating an environment that nurtures the spirit of the individual and is supportive of family and friends that may be struggling with these changes.
